Is it possible to always receive an notificayionn when a report is flagged by a reader? The only possible option i can see, is receiving an email when a comment has been autommoderated(5 downvotes). But that is too late.

Just change the value of the "Do auto-moderation if comment is flagged more than X times" option to 1. In this case, the "Notify admin when comment is flagged more than X times" option also will be changed.

Notify admin when comment is flagged more than 1 times.
