How can the form be customized to receive the input: "Please enter the code above to comment". And a numeric code next to it to fill in

We may suggest you use the old versions of wpDiscuz plugin (e.g. v. 5.3.6). More info here: https://wpdiscuz.com/docs/wpdiscuz-documentation/getting-started/custom-comment-form/comment-form-builder/
However, we don't support the old versions of the wpDiscuz plugin.

I use wpDiscuz 7. The picture was just an example of another website I found to describe the formular option
I understand that. The Captcha was removed started from wpDiscuz 7 versions. Currently, only ReCaptcha v2 is available. If you want to display it you should use the old version of the wpDiscuz plugin.
