how to make confirmation and email notification like this?

i mean how to add field "akan hadir" and "berhalanagan hadir" like the picture above.

so before user comment he must choose field like first square red box pict above, and after user choose field and submit comment. the field appear (like square red box second at the pict above)

You should customize the wpDiscuz plugin. Please follow this instruction if you decide to do it yourself:
