Please have a look at the screenshot. The options for displaying the form are missing. I habe a multisite and only one (the main site) has this issue. I don't know why. I have checked with another browser, cleared the caches and searched for options, but I guess it's not a feature... it's a bug.
This kind of issue is usually caused by either conflict with other plugins or the active theme. Please deactivate the plugins/theme one by one and check after each deactivation to find the problem maker.
Keep the cache plugin (if you have) deactivated during this testing process.
Don't forget to press Ctrl+F5(twice) on the page each time before checking.
