Any possible known cause?

In admin i go to wpdiscuz => Forms => Add New
Give the forma title and hit publish without touching anything else, page reloads and i got this.

Please confirm for us to deactivate the plugin/plugins if it's necessary. Is it the staging website so we can make some changes in the wpDiscuz plugin codes?

Please delete the File Manager plugin via the FTP plugin and let us know.
It would be better if you provide us with the FTP access for the current folder.
