The attach file button is only available for the post, page, attachments post types. For custom post types the button is not visible.

For custom post types you should use the wpDiscuz Media Uploader addon. More info here: https://gvectors.com/product/wpdiscuz-media-uploader/

The issue is fixed by our developers.

Could you please send the admin login details to infogvectors.com email address to allow the developers to check the issue? We don't find such an issue on our test websites. This is something specific in your case. 

Could you please deactivate the wpDiscuz and see if there are two comment forms or not.
