Please navigate to Dashboard > wpDiscuz > Forms admin page, edit the Default form and just click on the "Update" button.

Two issues were observed:

1-I have installed and activated the plug-in after the activation extra textbox i with insert button showing after the footer although the comments section shows perfectly (image 1)

 

2-Rating of article displays very large 5 stars on responsive vertically
